Privacy Policy

Generally, in Alaska, you have to fill in some state-approved forms when you submit your application for renewal or initial licensing. However, we do not always manage to get the required forms. So, the article is a small guide through the documents you need and their PDF formats.

Applications

If you want to get an Alaska Driver’s License/Permit or ID Card you have to fill in the D1 form.
Motorcycle riders need to submit the MOTORCYCLE LICENSE OR PERMIT form.
If you are going to use your vehicle for commercial purposes, you have to submit the COMMERCIAL LICENSE OR PERMIT form.
Alongside the above-mentioned one, you may need the CDL MEDICAL SELF CERTIFICATION.
No matter when you apply for Driver’s License, the DMV requires you to pass a background check, thus, you’ll need the FINGERPRINT BACKGROUND CHECK APPLICATION form.

Get your driving records you may need the following documents

For by-mail applications, you have to submit the Division of Motor Vehicles REQUEST for DRIVING RECORD form.
If you make the request for a company, check the COMPANY REQUEST (MULTIPLE RECORDS) form.
Accident records can be received after you submit the REQUEST FOR CRASH REPORT FORM.
INFORMATION COLLECTION
